Waiver

I hereby acknowledge that I am the owner of the pet(s) listed below and am seeking services from Treats - Canine Massage & Remedial Therapy Limited, a Certified Canine Remedial Massage, Laser and Hydro therapist (hereinafter referred to as "Therapist").

I understand that the services provided by the Therapist are not a substitute for veterinary care and that the Therapist is not a licensed veterinarian.

I have been advised to seek advice and guidance from a licensed veterinarian for any concerns or issues with my dog(s) prior to seeking any services from the Therapist.

I am aware that the services provided by the Therapist are not intended to diagnose or prevent any disease or condition.

I acknowledge that any services provided by the Therapist are based on my dog(s) and my own personal choices and preferences.

I understand that the Therapist is not responsible for any injury, harm, or loss that may occur during or after the services provided to my pet(s).

I waive any and all claims and causes of action against the Therapist, including but not limited to, claims for negligence, breach of contract, or any other claim arising from or related to the services provided to my pet(s) by the Therapist.

I hereby release the Therapist from any and all liability, including any and all claims, demands, damages, and causes of action of any kind or nature whatsoever, arising out of or in any way related to the services provided to my pet(s) by the Therapist.
